Our consultant nurse specialist can provide the family and client with an experienced oversight and assistance in the community, depending on their rehabilitation goal. Their expertise covers:


  • Specialist Respiratory management including tracheostomy care, tracheostomy training and weaning advice
  • Continence rehab and care
  • Wound care
  • Sleep
  • Medication support
  • Neuroendocrine function
  • Family training
  • Dietary services
  • Treatment of various co-morbidities, such as: diabetes, cardiac conditions, orthopaedic injuries, cancer, gastrointestinal challenges, etc.


In partnership with the wider MDT, they collaborate with case managers and the family to ensure medical needs are met.


Nurses often meet with families to provide education, training and support, medication administration and management, medical appointments, and recommended health practices. For patients who are transitioning from rehabilitation units or hospitals to closer to home, who still suffer from various long term health issues, referrals can be established for ongoing care needs.
